I agree that, to my knowledge, my dog is obedient and non-aggressive.

 

I understand that my dog must be up to date on all of their vaccines and that my dog must be on flea prevention and parasite-free. If my dog is flea or parasite-infested, I give permission to treat them if necessary.

 

I agree to indemnify and hold harmless Pampa Animal Hospital and staff from any and all liability of any nature and claims, including any damage my pet may cause to family members or third parties and/or injury, death, sickness, or damage of my pet, family members, or third parties may suffer during or after playgroup participation.
